How early should I arrive for my appointment?
If you are a new patient, or an existing patient with new insurance information or new photo ID (eg: Drivers License) we recommend that you arrive at least 15 minutes prior to the appointment so that you can provide this information for our records. If you have been seen before in the office and have no changes to either Insurance or photo ID, you need to arrive in time for your visit.

What insurance does your office honor?
We participate in nearly all major health plans in California. If you have questions regarding your current benefits, you may contact your Health Plan by dialing the 800# located on the back of your health insurance benefit card. Here is a list of major plans we accept: Medicare, Anthem Blue Cross, BC/BS, Aetna, Cigna, Blue Cross, Blue Shield, HealthNet, United Healthcare, AARPt.

What should I bring for my first office visit?
Bring a list or all your medications with the name of the medication, the dose and the frequency you take your medication.

How do I get to your office?
Our main office is in Morgan Hill. The Morgan Hill office is on the East side of highway 101 off the Cochrane Ave exit. Our building shares open parking with the De Paul Health Center. Select the ‘Contact’ menu option and generate directions to our office from your location.

Do I need a referral to make an appointment?
You do NOT need a referral to make an appointment. Pease call your insurance carrier to confirm. If you have any questions, please call our office and we can assist you with this.

Are my medical records kept private and confidential?
Your medical records are handled with the utmost respect for your privacy and in full HIPAA compliance. Our staff are bound by strict confidentiality requirements as a condition of employment regarding your medical records. Ordinarily we will not release the contents of your medical file without your signed consent.

Can I request appointments, prescription refills and access my Medical Records online?
Yes you can! South Bay Cardiovascular Center offers these via our secure Patient Portal. You must web-enable your account with us during your next visit to the office before you can access it. Our front-office staff will verify some information to set you up for secure web-access to your medical records. A confirmation email with a secure link will be sent to you with a temporary passcode and instructions. Follow these instructions to access our Patient Portal.

I need a refill on a prescription, what should I do?
You can request a prescription refill securely via our Patient Portal several days before you actually need the refill. If you do not have access to your online account or have not activated it, please call the pharmacy several days before you need the refill and they will contact us or you can contact us directly. Be prepared to give all the necessary information about the drug and dosage.

I am having a problem and think I need to be treated, what should I do?
If this is a Medical Emergency, please call 911 or go to the nearest Emergency Room. If you are able to call the office during office hours and alert us of your problem, the Doctor or Medical staff will tell you what you should do and if necessary schedule a visit to see the Doctor.

What if I am having a problem or side effect with a medication?
If you think you are having a severe allergic reaction to a medication stop it immediately and call our office. If you or another doctor has stopped one of your prescribed medications call and let us know.

Is there anything special I need to know if I am on coumadin?
Coumadin is a powerful blood thinner. If you do not follow a specific diet your level may be affected. If you start an antibiotic, use alcohol or take higher doses of tylenol your coumadin level may be affected and you should contact our office. Remember it is your responsibility to contact our office if you have not heard from the nurse regarding your coumadin.

How do I receive my lab results?
The best way to receive your lab results are to access them securely online via our Patient Portal. If you have not web-enabled your account and have not heard from us within one week after you took your lab test, please contact our office.

I lost my pre-procedure instructions. Are they available online?
Yes they are available online. Please follow the Pre-procedure instructions link in the Patient’s corner menu on this webpage.
